What We Do

We wrap home health services around our patients to help them improve their health and wellness and remain in their home of choice. We work closely with a patient’s current care team to develop a coordinated plan of care. Where indicated, our services are covered through a patient’s Medicare Part A benefit which requires no co-pay. Services include:

Physical Therapy including programs in:

• Mobility and strength training
• Post-surgery rehabilitation

Occupational Therapy including programs in:

• Fall prevention and balance improvement
• Assistive device training
• Home accessibility evaluations and training

Speech Therapy including programs in:

• Cognition and memory improvement
• Post-stroke swallowing and eating retraining

Skilled Nursing including programs in:

• Respiratory monitoring and improvement for COPD
• New medication evaluation
• Diabetes  education and advanced diabetic foot care
• Wound care
